R formal → MISSING_DEFAULT_PARAM_DOC for each Missing<..> parameter
of inputs that has a formal default in defaults (keyed by Rust name,
as the per-parameter options are) and is no choice parameter of
per_param, whose own line names the choices. The key is the formal
formals gives the parameter, a declared formal included (#1842).
Missing<Quoted> / Missing<Quosure> count too: their default is never
evaluated either.
